For your convenience, following is an affidavit form for your proper
registration with your parish clerk of court (or in the case of Orleans
Parish, with the office of the state registrar of vital records). Register
at your earliest opportunity in order that you may be able to perform
marriage ceremonies when called upon. A suggested ceremony is available
from the Louisiana Archdiocese Episcopal Office, 5065 Florida Blvd, Baton
Rouge LA 70806.
