2 Introduction
INTRODUCTION
____________
To people with a mobility impairment, computer technology provides
an opportunity to gain independence in daily living and working
activities. The computer may, however, require modifications to
become accessible for an individual with a disability affecting
motor control.
Because impairments affecting motor control exist in differing
degrees of severity, it is necessary to consider all areas of
needs when choosing a computer for an individual with a physical
disability. Among the needs to be addressed are the degree of
mobility, positioning, endurance and communication.
The needs of some individuals may be met with modifications to
standard computer equipment. Others may need to replace an input
device or add software to tailor the computer to their specific
needs.
MODIFICATIONS TO STANDARD KEYBOARDS
Some people with physical disabilities can use a standard computer
keyboard, but may type with one hand, one or two fingers, a head
pointer, a mouthstick, a handstick or with reduced fine motor
control. For these situations, there are software and hardware
modifications that simplify keyboard operations.
Persons who have reduced motor control or who use a headstick or
mouthstick to press the keys may need some means of preventing
accidental keystrokes. A KEYGUARD is a keyboard overlay with
holes positoned over each key. It helps the user to stabilize
finger, hand or stick movement and to select a key without
accidentally activating others. Standard IBM keyboards have a
built-in AUTO-REPEAT feature. If a key is held down long enough,
it begins repeating and extra characters may be typed. Software
programs are available to slow down or eliminate this feature.
SINGLE-FINGER programs allow the user to change the operation of
keys such as SHIFT, CONTROL and ALT so that two or more keys do
not have to be pressed simultaneously. KEYLATCHES that toggle off
and on may also be used to lock down keys temporarily. For the
typist able to use only one hand, KEYBOARD REDEFINITION programs
allow each key to be reassigned and permit the keyboard layout to
be redesigned. For example, keys may be reassigned to distribute
the typing load more evenly between the fingers on one hand, and
keyboards with layouts for right- or left-handed typing are
available.
Introduction 3
ALTERNATIVES TO STANDARD KEYBOARDS
In some instances, substitutes or replacements for the standard
keyboard may be required. An individual who can press on a flat
surface but cannot depress keys may be able to use a MEMBRANE
KEYBOARD. There are EXPANDED KEYBOARDS with larger keys for those
who can target only large areas and MINIATURE KEYBOARDS for those
with an extremely limited range of movement of their limbs. Some
keyboards may be tailored to allow single keystrokes to produce
frequently-used words or phrases.
ALTERNATIVE INPUT METHODS
A keyboard is the most commonly used method of providing input to
a PC. For an individual with a motor control impairment who may
not be able to use a keyboard effectively with adaptations such as
those just described, other input methods have been developed.
The computer accepts characters or commands coming from these
alternative input methods just as if they were coming from the
computer's standard keyboard.
When choosing an alternative method for computer input, one should
make sure that it will work with all software applications to be
used, such as word processing programs, database programs and
educational programs.
One input method uses a SWITCH to enter the same data that would
be keyed. A wide variety of switches are activated by almost any
motion of any part of the body. A switch adapts to a user's
abilities - the voluntary actions that the user can reliably
control. For example, a switch may be turned "on" or "off" by
sipping and puffing on a tube, wrinkling the eyebrows or making
contact with a metal plate. A SWITCH INTERFACE, which can be a
software program or hardware device, acts as the link between the
switch and the computer.
One technique for operating a computer with a switch is SCANNING.
The user selects the desired characters or words from the computer
display or a separate panel. With scanning, a cursor or
highlighted area moves repeatedly from one character (or group of
characters) to the next. With several switch activations, the
selection is entered into the computer as if it had been typed at
the keyboard. Another technique using a single or dual switch is
MORSE CODE. The switches connect to an adapter that translates
the pattern of dots and dashes into standard keyboard signals.
The advantage of Morse code is that it may be faster than scanning
for some people and does not require a separate panel or occupy a
part of the computer screen.
With a DIRECT SELECTION method, the user "points to" the target
items through a single action such as eye gaze, head pointer or
light beam. The characters selected may be displayed on the
computer terminal or on a separate panel.
4 Introduction
VOICE RECOGNITION is an alternative input method by which a
computer accepts a spoken command. A computer can only do this if
it has a previously stored voice pattern to compare with the
incoming sound. With some voice recognition systems, the person
using the system must first train the computer by speaking several
repetitions of the key word or phrase into the microphone attached
to the computer. The computer saves this as a voice pattern. To
use a voice recognition system successfully, an individual should
be able to reproduce the same speech patterns that were used when
setting up the system; that is, the user needs to speak the same
way every time. When the computer receives voice commands for
input to the system, it matches these against the stored voice
patterns and executes the keystrokes that were set up for that
command.
ACCELERATION TECHNIQUES
In conjunction with the modified and alternative input methods,
other adaptations increase the rate of input from the keyboard or
an alternative device. ABBREVIATION EXPANSION allows the user to
input a predefined shorthand version of a word or phrase that is
automatically expanded by the computer into words. WORD LISTS or
WORD PREDICTION use the initial letter(s) of the words entered and
present a dynamic menu of word choices. The user then selects the
whole word, rather than typing each letter.
Acceleration techniques may be available as separate software
programs that run with an application program or as built-in
features of word processing packages or multi-purpose adaptive
software programs. In addition, acceleration features are
included in some programs that modify keyboard operation and in
software programs for alternate input systems.
ELECTRONIC COMMUNICATION AIDS
Individuals who have a physical disability that also affects the
muscles used in speaking may benefit from technology enhancing
oral communication. Portable devices providing synthesized speech
or written output may aid communication. They may be accessed in
a variety of ways, including touch-sensitive keyboards, switches
and optical pointers. They may be used standalone or, in some
cases, as alternate input to a computer.
ENVIRONMENTAL CONTROL
An environmental control program helps an individual operate
lights, a radio, television, a telephone or most any electrical
device. For example, by using an environmental control device and
a switch or voice recognition system, a person with reduced motor
control can turn appliances on and off. In addition, some
environmental control aids also function as input to a computer
for purposes such as word processing.
Introduction 5
6 Technology for Persons with Mobility Impairments
TECHNOLOGY
__________
Keyboard Modification
Many individuals with a disability can use a standard IBM PC
keyboard but may be under the constraint of typing with one hand,
with only one or two fingers, with a head pointer, mouthstick or
handstick or with poor motor control. Hardware and software
adaptations to the standard keyboard assist a person in achieving
productive, efficient and accurate typing on the computer
keyboard.
For persons who need some means of preventing accidental
keystrokes, KEYGUARDS are a common modification. A keyguard is a
plate which fits over the top of a keyboard and has holes over
some or all of the keys. It allows the user a place to rest the
hands while typing, but primarily it is designed to prevent
inadvertent key presses caused by poor motor control or other
factors.
Some other adaptations for the one-finger, one-hand or stick
typist are key latches and single-finger programs. Key latches
are levers positioned over keys such as SHIFT, CONTROL or ALT
which can be locked down on these keys. SINGLE-FINGER PROGRAMS
allow the user to change the operation of keys so that they do not
need to be pressed at the same time as another key. Keyboard
redefinition programs allow the value of each key to be reassigned
so the user can create a layout tailored to individual needs.
Software programs are also available to enhance a person's typing
speed and accuracy by reducing the number of keystrokes needed to
produce words, phrases, sentences or paragraphs. For example,
ABBREVIATION EXPANSION PROGRAMS can accept a few predefined
keystrokes from the keyboard ("macros" or abbreviations) and
expand them automatically into longer strings of characters
(user-defined). Word prediction programs work off the initial
letters of words as they are typed and present the user with a
dynamic "menu" of the most commonly used words that begin with
that letter. The user then selects the whole word from the menu,
rather than typing each letter of the word separately.

Keyboard Modification 25
26 Technology for Persons with Mobility Impairments
Alternate Input
The keyboard is the primary means of input to an IBM PC for most
people. For a disabled person, an alternate input method may be
needed in order to access the computer.
One option is a keyboard which is a different size from the
standard keyboard. Generally, these ALTERNATE KEYBOARDS have a
programmable membrane surface that allows the user to define the
key assignments and the size of the key.
Other ALTERNATE INPUT devices allow an individual to indicate a
desired letter or position on a matrix or grid by pointing with a
light pointer or touching the grid. Devices with scanning arrays
allow an individual to make a selection when the desired letter,
number or word is reached. Common forms for scanning arrays are
menus on a computer screen, grids of squares with lights which
light sequentially and dial faces (like clocks) with pointers
which revolve to point to symbols. When using a scanning array,
the user activates a switch to move through the choices to make a
selection.
In order to connect some alternate input devices to an IBM PC, a
keyboard emulating interface may be required to allow the device
and the computer to communicate. A keyboard emulating interface
translates the signals from the alternate input device into
signals resembling those from a standard keyboard.
An additional alternate input system is Morse Code entry using
single or dual switches. Keyboard emulators accommodating Morse
Code input and training programs are available.

PRODUCT: LONG RANGE OPTICAL POINTER
VENDOR: Words Plus
P.O. Box 1229
Lancaster, CA 93534
(805) 949-8331 ˙
PRODUCT DESCRIPTION: Long Range Optical Pointer is a hardware and
software package that allows keyboard input using only head
movement. The program creates a graphic image of the computer
keyboard on a second monitor. The user wears a headset with an
optical sensor device built-in. By directing the sensor toward
the screen, the user moves a dot around the keyboard image.
Holding the dot on a particular "key" for a certain (adjustable)
period of time sends that character to the computer just as if it
were standard keyboard input. Activating a SHIFT, ALT or CONTROL
key once holds that key for the next character only; activating
twice in a row latches that key on until it is released by being
activated again. The optical sensor may also be attached to
another part of the body if desired.

Alternate Input 51
52 Technology for Persons with Mobility Impairments
Switching Device
For most of us, the keyboard is the primary means of input to an
IBM PC. For people with little or no use of their hands, the
standard keyboard is not an effective means of accessing the
computer.
SWITCHES, when used in conjunction with a hardware or software
device providing input to the computer, allow a disabled
individual to enter information into a computer. Switches connect
to devices such as alternate keyboards, environmental controls and
electronic communication aids.
A wide variety of switches are available. In most cases, a switch
is available or can be designed to be used in the area of the body
or muscle group where the individual can produce an intentional
movement or action. Examples of how switches may be activated
include pushing small buttons or large plates, sipping and puffing
on a tube, wrinkling the eyebrows, blinking an eye or making
contact with a metal plate. The many types of switches available
make it possible for a person with limited or uncontrolled
movement to operate a computer.

a mobility-impaired person are included in this list. For
example, products to improve the key entry process by "guessing"
the word or phrase after just a few letters are entered. And, of
course, many word processing products today include spelling
checkers - a function helpful to many users.

112 Technology for Persons with Mobility Impairments
IBM PC Voice Activated Keyboard Utility
The IBM PC Voice Activated Keyboard Utility program uses the voice
recognition component of the IBM Personal Computer Voice
Communications Option. It allows control of well-behaved IBM PC
Application programs through voice commands. Command Vocabularies
(called overlays) must be developed for each application. Overlay
developers must have DOS familiarity and programming aptitude, as
well as knowledge of the application which they wish to drive with
voice commands.
DESCRIPTION
The IBM PC Voice Activated Keyboard Utility works with unmodified
well-behaved application programs and provides voice recognition
capability for commands and data input for these application
programs. The spreadsheet or other application program a user is
already using were not designed for voice control. The IBM PC
Voice Activated Keyboard Utility works with these applications by
turning spoken commands into keystrokes which are sent to the
program. The program receives these keystrokes as if they had
been typed at the keyboard and executes the keystroke sequence
accordingly. The program is not "aware" of the Voice Activated
Keyboard Utility's presence. With this utility, an application
program can receive input from spoken commands or typed commands.
Users must provide to the utility the following information to
work with application programs:
o The voice commands that may be spoken during execution of the
application program
o The associated keystroke sequence of a voice command that is
to be sent to the application program
o The rules for which commands are allowed at a given point in
the application program
The IBM PC Voice Activated Keyboard Utility application overlay
capability allows creation and loading of application specific
command vocabularies for the IBM Personal Computer.
Certain application programs that run on the IBM Personal Computer
include a plastic overlay for the keyboard. Such an overlay
reminds the user what functions are initiated by specific keys for
the application program. Command Vocabularies are called
overlays. The IBM PC Voice Activated Keyboard Utility Application
Overlay serves in a similar role as this plastic overlay, but for
voice commands. The user may request IBM PC Voice Activated
Keyboard Utility to display a list of voice commands to be spoken.
IBM PC Voice Activated Keyboard Utility 113
In addition, the IBM PC Voice Activated Utility application
overlays include "grammar" support. "Grammar" provides rules
about which voice commands are allowed at a given point within the
application program. For instance, a spreadsheet program might
have a list of over 150 commands, functions and options. However,
at a given point within that spreadsheet, perhaps, only some of
these are considered "legal" commands, functions and options.
Once an application program and its respective IBM PC Voice
Activated Keyboard Utility application overlay are installed, the
user trains the utility to the user's speech patterns for the
vocabulary words in the overlay. The utility includes a training
interface for this purpose.
Users may gain temporary control of the display and keyboard and
may invoke IBM PC Voice Activated Keyboard specific functions such
as:
o Train new commands, retrain old commands
o Define new command names and associated keystroke sequences
o Edit command names and associated keystroke sequences
The IBM PC Voice Activated Keyboard Utility program diskette
contains partial samples of voice command overlays for some IBM
programs. These samples are for illustration only and do not
imply that these applications will receive productivity
improvement from voice commands. They also may not provide the
level of vocabulary required by all users of the programs. The
Keyboard Utility may be used to customize these vocabularies for
individual use.
Sample overlays are provides for the following applications: IBM
Assistant Series: Filing Assistant, Reporting Assistant, Writing
Assistant, Document Retrieval Assistant; IBM PC DOS; IBM
DisplayWrite 3; IBM Personal Decision Series: Data Edition, Plans
+ Edition, Plans Edition, Reports + Edition, Words Edition; IBM
Personal Computer Multiplan(TM) by Microsoft.
